The title page should include the following: (1) the full title of the manuscript; (2) the full name of each author, including the highest earned degree; (3) the institutional affiliation of each author together with the city and state; (4) acknowledgment of the source(s) of support for study, if any; (5) the full name and address of the author to whom correspondence should be sent.

Illustrations and Photos                                            Submit the most original artwork available (three copies).  High-quality laser prints are also acceptable.  Prints should be submitted in black and white.  Do not place tones of any kind behind graphs or charts.  Color artwork can be published only at the author’s expense.

All illustrations should be identified and numbered using Arabic numerals.  Identify top of illustration with up arrow.  Also label illustration with author’s name.  All legends for figures should be typed on a separate sheet.

If illustrative material which has been previously published is to be included with the manuscript, it is the author’s responsibility to obtain written permission to use this material.

Tables                                                                           Number tables in the text in consecutive order using roman numerals.  Type each table double-spaced on separate pages.  Each table should have a title and each column properly identified.  All abbreviations used should be expressed as a footnote to the table.  Do not use internal vertical type material.

References                                                                References should include only articles cited in the text.  They should be consecutively numbered as they appear in the text.  When a reference contains more than three authors, list only the first three authors followed by “et al.”  After the author’s listing supply the full title of the manuscript in lower case (except where a capital is required), the journal title abbreviated according to Index Medicus, the volume number, the inclusive pages and the year of publication.  Book references should include: author, chapter title, book title, editors, place of publication, publisher, year of publication, and inclusive page numbers.

Electronic Versions                                                   Papers should be submitted the same as “Preparing issue” text to be saved as “Word document PC or MAC.”  Figures should be sent separate from text version sent as “TIFF.”  Note to authors: Any figures sent from Power Point or saved from Power Point are not acceptable for high-resolution printers.

Status of Manuscript                                                The receipt of your manuscript by the Journal will be acknowledged and a decision regarding its status made as soon as possible.  All manuscripts are subject to editorial review.

One set of page proofs will be sent to the corresponding author for review prior to publication.
